• Do you have a loved one living too far from you to take proper care of them?
• We can assign a Registered Nurse to visit your loved one in their home or living facility on a daily, weekly, or as-needed schedule.
• Do you work full time and need someone to take your mother, father, or loved one to their doctor’s appointments?
• We will provide you with a Registered Nurse to take your loved one to their doctor’s appointments, accompany them during the visit, and then provide you with a full and accurate report of your loved one’s health assessment. Our RN will work directly with your loved one’s physician to ensure all required treatments and medications are followed in accordance with the physician’s instructions.
• Has your loved one been hospitalized, and you need support visiting them or transitioning them from the hospital to their home or facility?
• Our RN case management program will provide a Registered Nurse to make all the necessary medical and in-home health care arrangements to ensure a smooth transition from their hospital stay to their home, including medication management, occupational and physical therapy needs, and rehabilitation equipment ordering, and more.
• Is your senior loved one having trouble staying safe in their own home or driving a car?
• Our RN Case Management will provide a safety evaluation to properly assess your loved one’s ability and help prevent them from falling or having to be hospitalized.
• Does your senior loved one have cancer or another condition that requires medication management regularly?
• We will assign one of our RNs to stay with your loved one, administer the proper medication at the proper and prescribed doses for as long as your loved one needs it.
